Check-in is a bit confusing. There are separate desks for ticketing and check-in. The US carriers are at the number 12-15 check-in counters through an additional layer of security before you go through the gate security. The posted open times for the ticket offices appear to be guidelines only.

Malpensa Terminals 1 And 2
Terminal 1 international is large and clean, with decent restaurants, stores and bathrooms. Terminal 2, however, is small and lacking in facilities (no post office, no railway station, few shops).

Not Child Friendly!
If you have a child, or even worse, a baby forget about changing them at the airport.

The toilets in the International area are relatively clean but in the National area, where you transfer to flights within Italy, the toilets are filthy beyond belief.

We ended up changing our baby on the overcrowded benches of the waiting area and dumping the byproduct in the nearest bin (not that it would make any difference had we thrown it on the floor given the general cleanliness of the area).

Malpensa Airport
A large airport. One last chance to buy something, but be aware of the prices which are much higher than in the city of Milan. Lots of restaurants from Italian to international food. It will take about 45min from the city to the airport in one of the many non-stop bus companies you can find near the central station.

Milan-Malpensa Train
To go between the Malpensa Airport and central Milan, you can take the train called the Malpensa Express. The ride takes about 40 minutes. The train departs every half an hour from early in the morning until around 9:00 at night. After hours a bus is provided once an hour until around midnight.
